Pick up your Federal and New York State Tax Forms in the Reference Room at the Port Jervis Free Library. We have commonly requested forms including:

• Forms 1040, 1040A, and 1040EZ (with instruction booklets)
• Earned Income Credit
• Child and Dependent Care Expenses
• Itemized Deductions, and others.

If the library is out of a particular form or you need a hard-to-find form, you can print any form from the 3-ring notebook or the IRS website. The cost to print at the library is twenty cents per page. The Port Jervis Free Library is open from 10 a.m. to 9 p.m. on Mondays, Tuesdays and Thursdays; 10 a.m. to 6 p.m. Wednesdays and Fridays; and 10 a.m. to 5 p.m. on Saturdays.
